Tramadol (Ultram)
Tramadol (Ultram) is often considered a “step” between non-opioid relievers (like Ibuprofen) and stronger narcotics (like Morphine). It is converted in the liver by the CYP2D6 enzyme into a much more potent metabolite ($M_1$). Because it increases serotonin levels, it must be used with extreme caution in patients taking antidepressants to avoid Serotonin Syndrome. It is also uniquely associated with a risk of seizures, even at recommended doses, particularly in those with a history of epilepsy or those taking certain other medications.
